VBD Accident Report *- This form must be filed by the Volunteer Building Director whenever an accident occurs in a gym that requires extensive first aid or calling for emergency medical treatment.
VBD Damage Report *- This form must be filed by the Volunteer Building Director whenever there is damage to any school equipment.
VBD/Facility Director Bi Weekly Form *- The bi-weekly report required of VBDs when checking into and out of a gym. It's purpose is to ensure the VBD does the pre-use and post-use inspection with a member of the custodial staff.
