This form is for parents or guardians to submit, starting in 2010, to receive a $10 rebate for each child, more than two in a family, participating and correctly registered to play VYI basketball.
County or House coaches may evaluate any person they are required to deal with because they are a coach of a VYI basketball team. The results of the evaluation will be forwarded to the Commissioner of the league in question for further action as deemed appropriate by the Commissioner and league officials.
This form is required before any commissioner will consider a request for a specific teammate and must be completed prior to November 1. ONLY ONE (1) NAME MAY BE ENTERED AND ONLY ONE NAME WILL BE CONSIDERED! If either the player name or the requested name has not registered and paid in full, the request will be removed from consideration. If both families do not make the same request a single request will not be considered. Although league commissioners will attempt to honor teammate requests, VYI's overriding concern is balanced teams.
This form is used whenever a waiver of all or part of the fees for basketball is requested. Prior to filling out this form the parents must:
1) register online
2) both parents volunteer (If parents volunteer for Building Director, both must take the current test listed in the left hand menu under Building Director Test), (If parent volunteers for coach they must fill out the House Coach Application and contact their league commissioner who will verify same),or p
VYI relies on fees and donations from the families of its particiapnts to help run a successful sports program and to fund the various activities the club supports.
This form is to be filled out in its entirety by any parent of a County player who wishes to comment on the quality of coaching they are receiving during the current County basketball season.
